The process of purchasing health insurance is very easy. The reason Anthem Direct is called an "individual” health insurance plan is because you are purchasing the medical health insurance as an individual versus a business.

Anthem offers “individual" medical plans that are available to individuals, couples, and families. Applying for Anthem is easy. For help or questions call (866)-508-0618

1. Complete and sign the enrollment/change application (2 pages). Be sure to select a primary care physician (PCP) for every family member applying for coverage. The easiest way to get your physicians code is to call your doctors office and ask them for their code with Anthem. (You can also access from this website.)

2. Complete all questions and sign the Individual Health Statement (2 pages). Be sure to answer the questions for all family members applying for coverage

3. Enclose a check payable to Anthem Blue Cross for the first months premium.

4. If you want to pay Anthem directly from your bank in the future, fill out the Electronic Funds Transfer Aurthorization (#7). Be sure to include a check or deposit slip marked “VOID”.

5. Mail all papers and checks to our approved Anthem producer: Creative Health Insurance, PO Box 2048, Manchester, Ct 06045

Once we receive your application, we will review your application for mistakes and omissions. A small omission could delay your application by a week or more. If everything looks OK, we will immediately send to the Anthem Direct underwriting department.
