Here are some tips and the application process.

1. Complete and sign the application.

2. Complete all questions on the health history.Be sure to answer the questions for all family members applying for coverage.

3. Be sure to complete and sign the underwriting authorization on page 5.

4. Enclose a check payable to "Aetna" for the first months premium.

5. If you want to pay Aetna directly from your bank in the future, fill out the Electronic Funds Transfer Form (page 6).

6. Mail all these forms and checks to the preferrred ConnecticCare producer:

Once we receive your application, we will review your application for mistakes and omissions. A small omission could delay your application by a week or more. If everything looks OK, we will immediately send everything to the underwriting department.
